We are continuing our recruitment for the Black Health and Social Services Hub

Canadian Mental Health Association Peel Dufferin (CMHA Peel Dufferin) is partnering with LAMP Community Health Centre (LAMP CHC) and Roots Community Services Inc (RootsCS) to design, develop and implement an integrated health and social services hub with and for the Black, African and Caribbean (BAC) communities in Peel Region.

The hub will provide primary health, mental health, addictions and social services programs that will holistically address the physical, mental, social and cultural needs of BAC communities, and seek to eliminate disparities and inequities.

We are looking for a Registered Nurse to join our team!

The Registered Nurse will report to the Program Manager, BHSS Multidisciplinary Team and will provide clients with a comprehensive care plan supporting both mental health and health needs, using a stepped care approach**. This support will improve client self-defined quality of life, reduce symptoms of mental health issues, improve health outcomes, maintain support systems, and allow bridging to other services. This position will work closely with other professionals through the provision of expertise regarding client medical needs and linkages between mental health and primary health care.







**Stepped Care approach is a system of delivering and monitoring mental health and addictions treatment so that the most effective, yet least resource-intensive treatment, is delivered first, only “stepping up” to intensive/ specialist services as required and depending on the level of client distress or need.

Status: Permanent full-time, 37.5 hours per week, evening hours and weekend may be required

Base Salary: $71,933.58 - $84,937.87 annually (Candidate qualifications will be considered when determining the starting salary)

Location: This position is based out of Brampton. Although we operate with a flexible workplace policy,





staff are required to have office and/or community presence as per program and client needs.

What you will do

- Provide comprehensive nursing assessment and plan nursing care requirements including monitoring vitals, completing medication reviews, monitoring results of treatment, health and psychosocial education, developing crisis plans and discharge planning
- Coordinate, schedule and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment
- Manage a caseload including regular re-assessments, updating risk levels and caseload reviews






- Collaborate with the multidisciplinary team to build a client care plan and provide short-term or intensive case management as needed
- Refer and link clients to community resources and groups that support recovery goals
- Contact primary care at regular intervals to report clients’ care plan and discharge summary
- Participate in regular staff meetings and care planning to provide nursing expertise for client care within multi-disciplinary team
- Facilitate groups as part of CMHA PD’s multidisciplinary team, including the creation of new groups and act as a back-up facilitator
- Support organizational nursing needs as required (e.g., annual flu clinic, mobile health clinic)
- Update all appropriate documentation within 24 hours of client contact, as required by CMHA PD policy and procedure






- Participate in the Nursing Community of Practice and other agency and community committees and workgroups as requested by manager.

What you need to join our team

- Bachelor of Nursing (BScN) and current registration as a Registered Nurse with College of Nurses of Ontario
- Minimum 2 years’ experience in the mental health and addictions sectors. Community / Primary Health Care experience is an asset
- The following experience / training is an asset with an expectation to attend training as they become available:
- Canadian Nursing Association specialty certification (community nursing, psychiatric and mental health)
- Medication Reviews / Best Possible Med history
- Medication Reconciliation
- Physical Health Assessment
- Virtual Care
- Interprofessional Multidisciplinary Teams
- Crisis Intervention Skills
- Concurrent Disorders







- Being knowledgeable in a variety of evidence-informed treatment modalities including anti-Black racism, anti-oppression and other culturally sensitive approaches to intervention.
- Demonstrated cultural sensitivity and ability to effectively work with Black, African and Caribbean communities.
- Demonstrated experience working within a multidisciplinary team and evidence of an understanding of the importance of partnerships and collaboration.
- Strong understanding of the current and historical health inequities experienced by Black, African and Caribbean communities.
- Demonstrated and practical understanding of anti-Black racism and intersectionality, and various barriers which affect diverse identities
- Intersectional understanding of current social realities and barriers experienced by BAC communities, from an anti-oppressive, anti-racist, feminist, self-aware and trauma-informed approach to practice is mandatory.
- Knowledge of local cultural programs and services to refer individuals and families from Black, African and Caribbean communities within the service catchment area.

- Strong interpersonal skills with proven ability to approach, listen, build trust, and communicate across diverse cultures and intersectionality. Willingness to continually improve understanding around cultural competence.

- Openness to operate from an anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional framework.
- Willingness to learn and apply practical understanding of different elements of diversity, such as gender identity/expression, race/cultural background, religion, sexual orientation, disability, immigration and precarious status, family status, etc.

- Fluency in a second language, including ASL (American Sign Language) is considered an asset.

- Valid Canadian driver’s license and access to an insured automobile in good
- A satisfactory Vulnerable Sector Screening (Police Check) report.
