The salary range for this position is CAD $49.20/Hr. - CAD $63.47/Hr.

Providence Health Care is currently seeking a Patient Educator for its Trancatheter Heart Valve Clinic at St. Paul's Hospital.

This is a Regular, Full-Time BCNU Level 4 position, and work hours are Monday to Friday, 7:30 to 15:30.

Reporting to the Operations Leader and working in collaboration with the Chief of Nursing for Professional Practice Issues, the Patient Educator:

- coordinates the day to day operations of a designated service and supervises staff
- acts as a clinical resource to nursing staff and interdisciplinary team members in the provision of direct care to patients






- assists in the promotion of research and quality improvement initiatives within the service
- teaches patients and families self-care, promoting their own abilities to develop and maintain healthy behaviours within the context of an illness or disease
- develops, implements and evaluates approved patient education programs

Education
Registration with the BC College of Nurses and Midwives as a practicing RN registrant plus three years of recent related experience or an equivalent combination of education, training and experience.

Your Day to Day:

The Patient Educator:

- Coordinates the day to day patient care activities of the designated service by performing duties such as:

- monitoring patient acuity and flow of patients in the service to ensure that staffing levels match patient and service needs; making or adjusting patient assignments as acuity of patient changes;
- supervising staff, actively monitoring and following up on the work of staff, coordinating work assignments, providing critical input on employee performance, orientating new employees, calling in relief staff as necessary, and authorizing overtime shifts; problem-solving and troubleshooting staffing issues;
- scheduling and coordinating the use of staff,





equipment and other resources;
- assigning the responsibility of care of patients to others;
- coordinating the care given by others in the designated service.

- Assesses patients/families physical, emotional and mental readiness and ability to learn through methods such as physical assessments, discussions, completion of questionnaires, chart review and through collaboration with other members of the multi-disciplinary team.
- Organizes resources, supplies and equipment for patient/family home. Develops educational materials for patient/family, such as pamphlets, brochures, instruction sheets and manuals.
- Teaches patient and or significant other(s) to administer treatments and operate equipment safely, both at home and in the hospital.
- Develops,





implements and evaluates patient education programs.
- Coaches patient and family to develop and maintain health promoting behaviours within the context of the illness or disease.
- Advocates for the patient and family through methods such as communicating patient and family concerns to other members of the patient care team. Intervenes as required.
- Provides direct nursing care, including physical assessments and administering medications and treatments.
- Monitors patient progress and adjusts plan of care to ensure patient outcomes by methods such as maintaining contact with patients at home and/or being the liaison with outside agencies.
- Participates in multi-disciplinary rounds and provides input regarding patient and family progress.






- Identifies trends in patient outcomes and recommends program modifications.
- Maintains and updates related records including patient treatment records and statistics.
- Promotes the use of research findings into practice through methods such as incorporating information into policies, procedures, reference materials, education materials and teaching sessions.
- Supports research activities and participates in the development of quality improvement initiatives as requested through methods such as the participation in ongoing qualitative and quantitative research projects and ensuring ongoing patient satisfaction measurement.
- Provides clinical consultation to internal and external health care professionals in area of expertise.






- Provides input to the Operations Leader regarding performance planning and coaching of staff. Acts as a clinical resource to unit staff in the performance planning and coaching process.
- Provides input and recommendations to the Operations Leader regarding equipment, supplies and staffing requirements. Advises the Operations Leader of concerns and problems and provides suggestions to resolve problems.
- Represents the program/service on hospital and community committees as requested, maintaining liaisons with external agencies. Reports back to program/service.
- Performs other related duties as assigned.
