The Addiction & Mental Health Navigator (AMHN) will provide a rapid response to individuals seen in the Emergency Psychiatric Assessment Treatment and Healing (EmPATH) unit at LHSC in the Emergency Department (ED). The unit differs from traditional emergency psychiatric care by combining a calming supportive atmosphere of a community crisis center and having the ability to support the most acute mental health needs. The AMHN will work collaboratively with an interdisciplinary team of social workers, doctors,





Nurse Practitioner’s and other clinical supports to assess, treat and refer individuals seen in hospital who would benefit from addiction and mental health supports in their communities.

This is a front-line, clinical position providing crisis intervention, brief transitional case management, system navigation, addiction and mental health support. This position provides support to individuals who need a different type of ED support in order to reduce avoidable, unnecessary and repeat emergency department visits.

Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed in the last six months. Interested applicants who reside in the City of London and are in need of a PVSC are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 3 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING:

- What you'll be making - Starting rate for this role is $26.74 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($29.57 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($33.90 per hour for non-regulated, and $36.71 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement







- When you'll work - 70 hours over a bi-weekly period; Monday-Friday 10:00am-6:00pm.

HOW YOU WILL BE MAKING A DIFFERENCE:

- Establish a caring and therapeutic rapport with patients by demonstrating trust, respect, and honesty and support inclusion and equity embracing diversity and addressing oppression
- Work effectively within a multidisciplinary team comprising professionals from various fields such as mental health, addictions, community supports and medical services






- Collaborate in open and effective communication when engaging in problem-solving and decision-making processes to address complex participant needs with the goal to ensure comprehensive and cohesive care for participants
- Share in the provision of face to face Entry / First Contact / Intake Services
- Provide intervention and short-term support to individuals experiencing an addiction or mental health crisis
- Facilitate early linkage with internal programs and/or other agencies and/or supports i.e. family, neighbours etc. by communicating and coordinating relevant information and providing case management services until linkages are created
- Provide screening for mental health and addiction issues and assessment of appropriateness of service ensuring the assessment incorporates cultural and ethnic factors






- Provide and facilitate referrals to addiction and mental health resources as appropriate
- Provide referrals to all appropriate social and health related services based on participant wishes
- Offer harm reduction strategies as well as abstinence strategies based on participants intentions and stated needs
- Provide discharge planning and support
- Deliver appropriate level of navigational support based on client capacity
- Share information about health and community/social services and link participants to same
- Provide assessment, treatment planning, referral, advocacy, preliminary case management, community treatment and education as appropriate to persons who have concerns about their own substance use
- Engage in advocacy and system transformation as appropriate
- Engage in community development, community awareness,





education and outreach as designated and appropriate
- Attend and actively participate in team meetings and case conferencing
- Maintain awareness of and adherence to agency policies and protocols as well as hospital policies and protocols
- Complete all required training as required by agency and hospital partner as required for the role
- Support and participate in the quality enhancement activities of the agency
- Actively participate in the supervisory process with immediate supervisor and site supervisor
- Provide coverage for other team members as volunteered/directed
- Other duties as directed by supervisor

WHAT YOU'LL NEED TO APPLY:

We require the following qualifications:

- Post secondary education in a related field e.g. BSW, MSW, post-graduate diploma in addictions or equivalent required (primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Ability to attend work regularly

We prefer the following qualifications:







- Clinical work experience ideally in substance use assessment and treatment including intake services, referral and treatment planning
- System navigation, coordinated care planning and discharge planning
- Individualized screening, assessments, assessing risk, treatment planning and transition planning
- Experience and detailed knowledge of serious mental illness usually gained by several years of working with this population
- Experience in working in a community-based setting with an extensive knowledge of social assistance programs and both generic and specialized local social support networks
- Experience in crisis intervention/management and crisis resolution
- Experience working with individuals with an ABI or Development Disability
- Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), the College of Psychologists of Ontario (CPO), or the College of Nurses of Ontario (CNO)
- First Aid & CPR, ASIST, and CPI certification
