Job Description

Salary Range: $47.29-$56.30

Position Summary:

Reporting to the Director, Medical Programs/Integrated Care Manager, the CMH@Home Patient Navigator is responsible for collaborating with patients and their families to develop high-quality, timely, and cost-effective individual service plans based on patient needs. Utilizing a multidisciplinary approach, the goal is to achieve optimal health outcomes.

The primary purpose of this role is to assist patients in reaching their highest level of functioning and independence, in alignment with their values, priorities, capacities, and preferences for care.





The Patient Navigator will determine eligibility for services in accordance with relevant policies and procedures and care pathways.

The Patient Navigator will provide comprehensive clinical services to patients and their families. This role includes crisis intervention, emotional support, consultation, counseling, completing referrals, and connecting patients to appropriate community services. The Patient Navigator will lead efforts to educate and counsel patients and families while facilitating referrals to support services, all aimed at addressing patients’ goals.

The Patient Navigator will liaise between the hospital care team including physicians, primary care and community support services partners including Ontario Health at Home.

Qualifications and Experience:

- Regulated health care professional holding current registration with a regulated College in Ontario as a:





Registered Nurse, Occupational Therapist, Physiotherapist or Speech Language Pathologist, or College of Social Work, Dietician
- Appropriate university degree or a suitable combination of education and direct experience
- Acute care experience of 3-5 years preferred
- Must be able to determine capacity in accordance with the Health Care Consent Act (1996)
- Knowledge of available community resources to support a safe discharge from hospital
- Ability to work independently
- Employee must demonstrate basic computer skills and literacy
- Proficient in utilizing computer applications to document, input and extract data accurately, receive and send emails, as well as in making calendar appointments






- Excellent communication skills and ability to facilitate discussion towards conflict resolution Commitment to maintaining and promoting healthy working relationships
- Ability to work both independently and amicably in group situations
- Demonstrated skills in the areas of organization, communication, interpersonal relations, leadership, decision making and problem solving
- Ability to apply best practice modalities for assessment and treatment within professional scope of practice
- Comprehensive appreciation and commitment to inherent values and ethics within the profession
- Solid ability to use MS Office applications (e.g., Word, Excel, Outlook, PowerPoint, etc.) and internet research skills and computer navigation
- Ability to analyze information, problem-solve and make good decisions
- Accountable for own actions and decisions,





making decisions within the scope of the position and referring issues/problems/events to the Integrated Manager OH@Home or Director of Medical Programs as required
- Solid documentation skills (clear, thorough, accurate and timely)
- Self-directed with the ability to organize, prioritize and multi-task
- Flexible, adaptable and responsive to change
- Detail oriented
- Dedication to working in a team environment, collaborating with all staff in hospital departments
