OVERVIEW

Community Outreach Services (COS) works in partnership with stakeholders in the community to provide mental health services in response to the needs of its clients, families and the community. The service is composed of the COMPASS (ACTP), REACH and Family Support Program. The Inpatient program provides intensive psychiatric care to those requiring more acute support.

The Occupational Therapist (OT) performs specific standardized assessments of cognitive and functional living skills, ADL and IADL assessments, and mental status assessments and provides consultation and occupational therapy services to address the cognitive, physical,





psychiatric and psychosocial needs of the patients in order to support their recovery and wellness. The OT facilitates group therapy and individual counselling in the outpatient and inpatient services.

PRIMARY RESPONSIBILITIES

PATIENT CARE AND COORDINATION OF SERVCE DELIVERY:

- Provide consultation and OT specific assessment of clients’ physical health, mental/ emotional health, cognitive ability, psychosocial status and/or functional status as it relates to performance in the areas of self-care, productivity and leisure in both the inpatient and outpatient settings. Assessments include, but are not limitedto: functional assessments, mobility/ gait assessments, seating assessments, falls risk assessment, ADL/IADL assessments, cognitive assessments and community safety assessments.

- Develop individual and group interventions to address the cognitive, physical, psychiatric and psychosocial needs of the patients in order to support their recovery and wellness based on the analysis of assessment findings and clinical reasoning. Interventions include, but are not limited to:





addressing seating and mobility needs, recommending home safety equipment, providing cognitive impairment support and cognitive rehabilitation, providing psychotherapeutic support, providing psychoeducation, safety and comfort planning, risk management, goal planning, skill building,community integration/linking clients to community resources and supporting engagement in meaningful activities. Monitors change in the patient’s functional status and adjust treatment programs and interventions accordingly.

- Educates patients, appropriate family members, students and other health care providers with regard to Occupational Therapy and the management of patient’s health problem
- Function as an integral part of the core Coordinated Care Team,





participate in and contribute to inter-professional care planning. Participates in inter-professional rounds and huddles using specific theory-based and skills of an occupational therapist, be available to the team as a resource.

- Provides OT specific assessment during the intake process of new clients on the COMPASS team.

- Facilitates group therapy and individual counselling sessions in the outpatient and inpatient services.

OTHER DUTIES

- Support a healthy workplace that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements
- Orients and supervises Occupational Therapy students
- Participate in research, program evaluation and quality assurance activities






- Attend and contribute to educational rounds within the Mental Health Service.
- Assist in the development and updating of therapeutic group programming for the mental health department
- Attend staff meetings and take part in policy and procedure reviews as necessary.
- Participate in program evaluation and development, including the collection of statistics.
- Attend occupational therapy and Mental Health Service Councils and meetings.

EDUCATION

- Graduate of an accredited university program in OT.
- Minimum B.Sc (OT) or equivalent required, Master’s degree in Occupational Therapy is preferred.
- Current registration in good standing with the College of Occupational Therapists of Ontario and membership with Ontario Society of Occupational Therapists.






- Formal training in CBT/DBT/Solution Focused therapy/Family Therapy/Supportive Counseling is would be considered an asset.

QUALIFICATIONS

- Minimum 2 years of recent relevant Occupational Therapy experience (within the past 5 years) working primarily with adults who have mental health and addiction issues.
- Demonstrated clinical experience in Psychiatric Occupational Therapy, with a minimum of 3 years’ experience required.
- Demonstrated experience utilizing Individual and group counseling skills, including expertise with Cognitive Behavioural Therapy (or Dialectical Behavioural Therapy), Solution Focused therapy, and Family Therapyrequired.
- Demonstrated familiarity with psycho-education, community resources, wellness,





skill development, vocational support, goal setting, community integration and illness management essential.
- Demonstrated Group therapy experience including, but not limited to, psycho-educational, task, recreational and psychotherapeutic groups required.
- Experience working in a client centred, recovery-oriented framework preferred.
- Experience with family support and psycho-education regarding mental illness and addiction preferred.
- Assessment of mental status skills required.
- Program planning and development experience preferred.
- Familiarity with the Mental Health Act.
- The ability to work with clients of diverse ethno-racial and culturalbackgrounds is required.






- Proven ability to effectively liaison and link with community resources.
- Excellent communication and interpersonal skills.
- Effective organizational and analytical skills.
- Proven interest in self-development and in the facilitation of development of others.
- Excellent leadership skills.
- Ability to involve/ empower patient/ family in their own care and to support clients' optimal independence in the least restrictive environment.
- Experience working with clients with a personality disorder and Dialectical Behavioural Therapy an asset.
- Good work and attendance record required.
