Day Shifts: 0800-1600, 0830-1630

Pay Band: Band-K $28.91-$32.04

Location: Hotel Dieu Hospital Site

PRIMARY FUNCTION:

The Clinical Intake Coordinator is a key member of the multidisciplinary team who provides services in a manner consistent with KHSC Mission, Vision and Values and that reflects the core values of Patient and Family Centred Care (dignity, respect, information sharing, participation, and collaboration); support a healthy workplace that embraces diversity, encourages teamwork. Reporting to the Program Manager, Mental Health & Addiction Services, the Clinical Intake Coordinator (CIC) assures streamlined access through a common entry point to intensive and specialized services which are patient-centred and fully integrated.







The Clinical Intake Coordinator is responsible for the effective coordination of the Centralized Intake Services, assigning and linking patients to the most appropriate mental health and addiction services and providers; facilitating redirection of referrals to community agencies and other specialized mental health resources and psychiatric facilities. The incumbent assures streamlined access through a common entry point to intensive and specialized services which are patient-centred and fully integrated. The incumbent provides initial screening and assessment, triage and assignment to services offered for individuals referred across the lifespan who suffer from various mental health, substance abuse and dependence disorders.





The incumbent acts as a consultant and clinical resource to patients, family members and referral sources.

PRINCIPLE RESPONSIBILITIES:

- Conducts comprehensive clinical triage and screening of all referrals in collaboration with MHAS nursing, involved family member(s), most responsible physician and other members of the multidisciplinary team and aftercare providers, within two (2) working days of initial referral;

- Complete detailed and comprehensive documentation during the referral process and updating documentation in the Electronic Management Record System (e.g. PCS, Lumeo); also works as an administrator to maintain and update the Client Record Management System (CRMS).







- Coordinates Central Intake Services and reception functions; ensures appropriate service coverage and access; services are provided dependably and reliably, consistent follow through with patient and referral source inquiries, requests or complaints, and are kept up to date.

- Assists the Program Manager with planning, developing and evaluating patient care by participating in the development and revision of policies, procedures and protocols.

- Participates in utilization reviews of regulated health professionals’ caseloads, developing recommendations to assure full utilization within scope of practice, staffing model adjustments to meet current and changing service demand and patient profiles; also maintains and tracks wait list times for various programs and services connected to Centralized Intake.

- Performs other related duties as assigned

QUALIFICATIONS:

- A diploma or degree in related field (e.g. Psychology, counseling Behavioral Science, Social Work, etc.)
- 3 years recent and relevant experience working in a clinical role within a mental healthcare setting
- In-depth knowledge of Ontario Mental Health Act, Health Care Consent Act, Powers of Attorney Act, Substitute Decisions Act and other relevant legislation
- In-depth knowledge of DSM-V diagnostic criteria and psychopharmacological-somatic therapies,





community-based and regional mental health and other relevant resources is essential
- Excellent interpersonal, communication, presentational and documentation skills with the ability to work well with patients, families, all levels of staff and community providers, developing strong internal and external partnerships
- Demonstrated strong mentoring, coaching and team building skills, ability to function as a change agent and within a leadership role
- Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care
- Exceptional time management, organizational and prioritization skills, with the ability to work well in stressful situations
- Familiarity with scheduling, word processing,





spreadsheet and database software
- Familiarity with mental health case-management software (CRMS)
- Capable of coping with a physically demanding workload, responding to medical and behavioural emergencies, ability to lift using appropriate lifting techniques and bend, push and pull using good body mechanics
- Demonstrated commitment to the mission, vision and values of Hotel Dieu Hospital
- Proven ability to attend work regularly.
- Satisfactory criminal reference check and vulnerable sector search required.
