Unit Summary

The Behavioural Health Program provides care for patients who have a diagnosis of a neurocognitive disorder, with associated verbal or physical expressions that cannot be managed in a lower level of care. The patient is admitted for assessment and treatment, with a goal of discharge, usually to a long term care environment.

As a member of the health care team,





the Social Worker provides direct patient care through the provision of psychosocial
screening and assessments; individual and family counseling; adjustment/coping; patient education; liaison and referral
to community agencies; advocacy and discharge planning. Services are provided through individual, family and group
counseling and by providing psychosocial expertise to the interdisciplinary team. The Social Worker participates within an
interdisciplinary team setting in education, research and the development of the program.

Schedule Work Hours

Monday to Friday; days.

This position is temporary until January 23, 2026.

1. Master’s Degree in Social Work from an accredited School of Social Work
2. Registered with, and in good standing with the College of Social Workers and Social Service Workers of Ontario.
3. Minimum 2 years experience with patients presenting with neurocognitive disorders and behaviours
4. Expert knowledge of community resources
5.





Experience in a geriatric setting preferred
6. Demonstrated clinical judgment and ability to utilize a holistic approach emphasizing quality of life
7. Excellent clinical, interpersonal, communication and organizational skills in an interdisciplinary team setting
8. Strong commitment to interdisciplinary care
9. Demonstrated commitment to ongoing learning
10.Strong assessment skills for patients and families with complex needs.
