The Role: Case Manager (Full-time)

The Maple is a permanent housing program that provides support to female identifying clients aged 25 and older who have experienced chronic homelessness. The Case Manager provides ongoing support and resources in a flexible, compassionate, encouraging environment, including being responsible for all aspects of the case management of the clients. This includes mental and physical health supports, employment and education, social inclusion and recreational activities, and in-depth work connecting clients with their natural supports.

This position has a 37.5-hour flex-week, that’s reflective of the needs of the clients and families,





removing barriers to connection and support (days, evenings, weekends as needed, minimum 2 evenings a week).

Key Tasks & Responsibilities:

- Provides intensive, timely and barrier free case management coaching/support to youth and their families/Natural Supports who are ending their experience with homelessness in accessing housing and basic needs, removing barriers to stability and teaching/coaching life skills.
- Provides case management activities that includes family and natural support reconnection, system advocacy, supporting ongoing connection to education and/or employment, culture and community.
- Support youth and families in their homes, community spaces, schools, etc.
- Work with youth and their families, both individually and collectively,





to negotiate boundaries, manage conflict, and improve communication all aimed at family reunification. This might include crisis resolution, mediation, safety planning, advocacy, facilitated referrals, and system navigation.
- Intervening and de-escalating crisis situations as they occur.
- Work closely with Youth Workers to ensure clear communication and collaboration in Supportive Program Planning and facilitation that is resident driven and are aligned with youth and family goals.
- Assisting with building life skills such as resume building and attaining educational resources, in addition to Arts, Recreation and Cultural activities that will help build self-esteem and connect them with their community.
- Responsible to complete all goal planning,





outcome reporting and client feedback tools as required.
- Completing intakes and documenting casework meetings, goal plans, safety plans, and Critical Incidents.
- Attending required training opportunities within the appropriate timeframe.
- Other duties as required.

Knowledge and Skill Expectations:

- Social Services education with recent and relevant experience, or an equivalent combination of training and experience.
- Understanding of child and youth development, family functioning, positive parenting and systems theory/ecological approach.
- Experience or in-depth understanding of Harm reduction and Trauma Informed Practice is considered an asset.
- Proficiency in Microsoft Office programs and ability to learn database systems.






- Concise and clear communication (verbal and written) and problem-solving skills.
- Up-to-date Standard First Aid and Level C CPR training.
- Valid Class 5 Driver's License, reliable vehicle and clear driver’s abstract as this position may require you to drive clients.
- Ability to provide a Criminal Record Check with vulnerable sector search and Child Intervention Record Check.

Reporting Relationship:

This position directly reports to a Trellis 24/7 Program Manager.
