We opened our new Ken and Marilyn Thomson Patient Care Centre in early 2023 providing eight floors of modern and bright care spaces and will continue to transform our campus in the coming years.

Our Transitional Youth Program within our Mental Health and Substance Use Services provides Social Workers a unique opportunity to grow their career and leadership experience. Here staff have the opportunity to improve the mental health and wellbeing of youth, from 16 to 29 years of age, by providing specialized youth-centered mental health and substance use care. We focus on the whole person, to support youth and young adults to cope, develop, grow, thrive and transform, leveraging their strengths. This dynamic, community oriented program has a team who are committed to learning and providing excellent care. The TYP is comprised of Social Workers, Peer Support Workers and Psychiatrists, providing trauma-trauma informed, person-centered, flexible and timely supports to youth –where, when, and how they need it.

This position offers an exciting opportunity to be part of an innovative and growing program,





working in both the Child / Youth and Adult Mental Health Services. This role is a combination of community-based, youth hub-based and clinic-based work. Applicants should be prepared to travel within the TEHN catchment to meet with youth. In this role, you will provide cross coverage to other mental health programs on an as needed basis.

What to expect:

As a member of the Transitional Youth Program, you will:
- Provide brief clinical support and navigation help to youth and young adults with mental health and substance use needs, aged 16- 29 years, during times of transition in services.
- Support youth who are transitioning from Child and Youth Mental Health Services to the Adult Mental Health system, by providing interim case management, therapeutic support, and identification of optimal adult services,





as required, during the period of transition in services.
- Support youth and young adults in the transition from in-patient or emergency services to the community, by providing interim case management, therapeutic support, and identification of optimal services, as required, during and after discharge from the original treatment setting.
- Develop and maintain close relationships with the hospital emergency department, and the child and adult in- and out-patient mental health services.
- Participate in the provision of clinical navigation and/or harm reduction services through the integrated youth hubs in the MGH catchment.
- Participate in consultation with both internal and external stakeholders around issues regarding youth and MH care at MGH.






- Develop and maintain close partnerships with a range of community stakeholders in the TEHN catchment area to ensure seamless and supported transitions between services.
- Support the continued implementation and evaluation of the Transitional Youth Program, in conjunction with the Manager, Child and Youth HealthServices, the MGH mental health and substance use services, community partners, and youth and family stakeholders.
- Participate in activities in collaboration with the East Toronto Ontario Health Team, Youth Wellness hubs, in the TEHN catchment area.
- Represent the Transitional Youth Program at internal and external stakeholders meeting.
- Sharing knowledge and expertise with others, while advocating for and promoting health and well-being of clients.






- Caring for your clients as part of a respectful, collaborative interprofessional care team.
- Working with the MGH team to advance your clinical competencies through ongoing program based and corporate educational and professional development opportunities.

EDUCATION

- Completion of a Master’s degree in one of the following: Social Work, Psychology, or Counselling Psychology.
- Registration with a regulatory body such as the Ontario College of Social Workers and Social Service Workers or the Ontario College of Psychotherapists required.
- Meet the qualifications to perform the controlled act of Psychotherapy as an Independent Practitioner.
- Completion of certificates in psychotherapeutic modalities including cognitive behavioural therapy, dialectical behavioural therapy,





motivational interviewing and solutions focused therapy considered a strong asset.

QUALIFICATIONS

- Minimum of 2 years’ clinical experience in a mental health service working with youth and young adults aged 16- 29, required.
- Minimum of 2 years’ recent experience in transition or navigation support, required.
- Minimum of 2 years’ experience anddemonstrated skill in counselling, psychotherapy, family work and crisis intervention, required.
- At least 1-year experience providing harm reduction counselling an asset.
- Relevant experience in hospital, community and home-based mental health settings required.
- Demonstrated experience and skill interacting with youth/ young adults presenting with a broad range of mental health conditions,





including serious and persistent mental illness, addictions, trauma and conditions associated with suicidality and /or aggression.
- Knowledge of motivational interviewing, engagement strategies with youth, preferred.
- Familiarity with marginalized populations, including ethno cultural and religious groups, LGBTQ+ youth and youth adults, those impacted by poverty and those involved in youth justice.
- Extensive knowledge of community mental health services for youth and young adults.
- Experience and skill in collaborating effectively with hospital, community, youth and family stakeholders and in building and maintaining solid working relationships with community partners.
- Ability to work both independently and as part of a team.
- Demonstrated knowledge of the Ontario Mental Health Act,





Consent & Capacity legislation, Privacy Legislation, Mental Health Reform and consumer/survivor issues.
- Excellent oral and written communication skills, including the ability to prepare reports and do presentations.
- Computer literacy, including MS Word and Excel. Experience with Cerner system an asset
- Excellent work andattendance record.
