Position: Nurse Practitioner (1 position - full time)
Pay: $62.66 per hour
Schedule: 37.5 hours per week, day shift
Contract end date: June 6, 2025
Anticipated Start Date: as soon as possible

Organization Overview

The 360 Degree Nurse Practitioner - Led Clinic (NPLC) is a dynamic community-based interdisciplinary primary health care clinic providing comprehensive primary health care services to individuals and families residing in Peterborough City and County who are unattached to other primary care providers.

The NPLC is committed to providing access to innovative, integrated primary health care services and programs delivered collaboratively with a focus on healthy equity and patient centeredness.





Located in the downtown core of Peterborough, we have expertise providing care to people whose health is made vulnerable by poverty, homelessness and poor access to the social determinants of health. We operate from a harm reduction philosophy. We are committed to working from within an anti-oppressive and anti-racist philosophical framework that is trauma-informed and which values cultural humility and inclusiveness.

Working within the interdisciplinary team, the Nurse Practitioner provides high quality, full scope professional care to patients, consistent with the vision and philosophy of the NPLC and with the standards of the College of Nurses of Ontario. The NP works in a 'Most Responsible NP' model of care.

We are looking for a Nurse Practitioner who:
Is passionate about working with people who are vulnerable: living in poverty; insecurely/un-housed; living with mental illness +/or using drugs; LGTBQ2S+; refugee; identifies as Indigenous






Is a flexible and adaptive person who actively seeks ways to contribute to care delivery within the primary care team
Is a critical thinker and creative problem solver
Is committed to working from a harm reduction philosophy of care
Believes in the value of client-centred, team-based care and enjoys working in an environment that empowers all members of the team, including the client, to contribute in meaningful ways
Has knowledge of relevant primary care practices and guidelines and means to adapt this knowledge to support marginalized populations.

Role and responsibilities to include:
Contributes to organizational activities which support the achievement of the NPLC's Mission, Vision, and Strategic Plan






Provide comprehensive collaborative primary care and services working within the full scope of practice as defined by the College of Nurses of Ontario
Participate in 360NPLC programs/projects, as appropriate, which may include:
Routine primary care including advanced access for low barrier access and emergency room diversion
Home Visits
Outreach
Substance Use Support and Treatment
Understand how to incorporate harm reduction practices in a clinical setting
Practice from a trauma-informed, strengths-based, anti-oppressive lens
Focus on social and structural determinants of health that may influence the client's health status
Support clients in being at the centre of their care, empowering them to make decisions on their health and goals and supporting patients managing their health and care






Participate in chart reviews, discussion of difficult cases, and client inquiries
Advocate for patients with colleagues, agencies, and institutions
Address stigma and reduce barriers to health and social care access wherever possible
Assure standards of patient care are maintained in accordance with accepted current best practice guidelines and 360NPLC policy.
Supervise and evaluate students of the 360NPLC, alongside other members of the team
Maintain professional development through continuing education, including learning opportunities provided at the 360NPLC.

Qualifications:
Registration in the RN, Extended Class in good standing with the College of Nurses of Ontario






Minimum 2 years of primary care experience working in community-based health care settings
Demonstrated strong primary care clinical assessment skills to full NP scope of practice
Exemplary oral and written communication and interpersonal skills
Experience working with vulnerable populations (such as people who are homeless, living on low income, newcomer, refugees, trans individuals)
Experience working with people experiencing mental health or substance use issues from a harm reduction philosophy
Experience prescribing controlled drugs, and Opiate Agonist therapy is an asset
Experience with mental health and addictions treatment/management is an asset
Experience with electronic medical records system is an asset
Demonstrated ability to advocate for patients to ensure health care system responsiveness






Satisfactory criminal reference check/vulnerable sector check
Current BCLS
Current Ontario Driver's License

Job Types: Full-time, Fixed term contract

Pay: $62.66 per hour
